Act as a primary point of contact for patients experiencing breast related issues, offering empathetic support, guidance, and encouragement throughout their treatment journey.
Provide comprehensive education on breast cancer diagnosis, treatment options, and available resources, ensuring patients and their families have the information needed to make informed decisions.
Address patient concerns, questions, and uncertainties, offering clarity and reassurance as they navigate the complexities of their diagnosis and treatment plan.
Care Coordination:
Collaborate closely with healthcare providers, including oncologists, plastic surgeons, radiologists, and other specialists, to coordinate seamless and integrated care for patients experiencing breast related issues, including breast cancer.
Coordinate with hospital personnel to manage surgical block times and OR schedules.
Perform insurance pre-authorizations.
Facilitate appointments, diagnostic tests, and treatment procedures, ensuring timely access to necessary services and interventions.
Assist patients in understanding and adhering to their treatment plans, including medication schedules, follow-up appointments, and lifestyle recommendations.
Advocacy and Empowerment:
Serve as a strong advocate for patients, ensuring their voices are heard and their needs are addressed within the healthcare system.
Resource Referral and Support Services:
Provides seamless referrals to resources to ensure elimination of barriers to care.
Offer guidance on available wellness and supportive care services, including physical therapy and integrative therapies.
Tracks and manages follow up for patients with identified needs as applicable.
Qualifications/Training:
Strong clinical knowledge of surgical patient management. Oncology experience is preferred.
